  • Enrollment Information & Important Documents

    We would love for your family to be a part of the SFLC family. If you would like to register your child at our center the following documents are needed for each child:


    • Completed Enrollment Form for each child attending
    • Copy of Birth Certificate
    • Copy of Shot Records
    • Copy of last Well Child visit or Physical
    • Signature Page of Family Handbook
    • Custody Paperwork if Applicable
    • Copy of at least one parent or legal guardian’s ID
