We would love for your family to be a part of the SFLC family. If you would like to register your child at our center the following documents are needed for each child:
Completed Enrollment Form for each child attending
Copy of Birth Certificate
Copy of Shot Records
Copy of last Well Child visit or Physical
Signature Page of Family Handbook
Custody Paperwork if Applicable
Copy of at least one parent or legal guardian’s ID
