Enrollment Information & Important Documents
We would love for your family to be a part of the SFLC family. If you would like to register your child at our center the following documents are needed for each child:
- Completed Enrollment Form for each child attending
- Copy of Birth Certificate
- Copy of Shot Records
- Copy of last Well Child visit or Physical
- Signature Page of Family Handbook
- Custody Paperwork if Applicable
- Copy of at least one parent or legal guardian’s ID
